Home Infusion Services
PROGRAM OVERVIEW
Access
Infusion Partners is a full service, regionally integrated
home infusion pharmacy service developed to enhance the
continuum of care for patients and physicians and to reduce
health care costs. These services include: injectable
medications and solutions, disposable infusion supplies,
infusion pumps and poles, product delivery and pick up, and
the professional and support services that go with it.
Access Infusion Partners is a joint venture of the Lee
Memorial Healthcare System and ProHealth Medical, Inc.
SERVICE AREA
Access Infusion Partners provides home infusion pharmacy
services throughout South West Florida. Affiliate home
infusion pharmacies are also able to cover the State from
Jacksonville down to Miami.
HOURS OF OPERATION AND STAFFING
Office hours are 8 a.m. - 5 p.m. Monday through Friday.
On-call personnel (pharmacist, nurse and courier) cover
after hours and weekends.
Core staffing consists of pharmacists, nurse coordinators,
pharmacy technicians, reimbursement specialists, couriers,
and office personnel.
The pharmacists are responsible for the pharmacological care
of the patients and work directly with the patients'
physician(s) to ensure patients receive what they need. In
conjunction with the physician, patient and family, and
other health care team members, the pharmacist develops a
plan of care for the patient and monitors the progress
toward the goals of therapy.
Our nurse coordinators play a dynamic role facilitating care
for patients across the continuum, working directly with
staff in the hospitals, physician offices, home health and
hospice programs.
